The collision happened on the M40 in south Oxfordshire on Monday afternoon.
Shortly before 4pm, police received reports a Subaru Forester towing a caravan was travelling in the wrong direction on the northbound carriageway of the motorway.
The vehicle was subsequently in collision with two other cars, a Ford Mondeo and a Ford Focus.

A 55-year-old man died Monday morning after apparently suffering a medical emergency while driving, which caused his car to shoot off the road and slam into a gas station convenience store in Lawrenceville.
The incident, which occurred around 10 a.m. at the Exxon station on Lawrenceville Highway, also injured a woman who was struck by the car as she stood outside the store, according to Lawrenceville Police Department Lt. Jake Parker, spokesman for the department.
